I'm not sure. Maybe it's when the admins are deleting a post or editing something. I know that when we view the moderation panel, it says "unknown action".

"Nothing you can see" I believe shows when people (particularly admins or moderators) view parts of the forums we can't view, such as the admin panel or the beta tester board. "Unknown action" is just an action that the online list doesn't recognize.
