I was watching the IndyCar race (don't kill me! :P) when I tuned to TWC and saw this. The HD feed didn't even get the teasers; it stayed on the Studio 1B shot while EE was starting (you could see EE on the in-house TVs), and it didn't switch over to EE until Adam went to the key wall.

What happened is that master accidentally sourced its own production system instead of the "full" control room. (note the plain blue LIVE tag in the upper left-hand corner). They probably switched the SD feed over to full control first, then switched the HD feed over later.

They really lost it during the 3:40 PM CT hour of WC. When Jeff was at the key wall, they accidentally displayed nothing but a black screen on the key well for several seconds; then, after Jeff was done, he tossed to Vivian at the desk, and the camera suddenly started rotating to the left, focusing on the empty seat next to Vivian for the entire time that she was talking.

I do sometimes, but before they switched to the big desk I didn't really pay attention to those things. ;)

Did anyone see what happened at the start of EE? They started about a minute early, and it apparently did not start as planned. It stayed at the black screen for several seconds, then went to an emergency Sat LF for about 30 seconds, and then the EE opener/teaser played.

On a related note, I notice these kinds of issues tend to arise when EE Weekend starts early (which happens most of the time).
